Top-grain sits just below full-grain in the usual ranking of leather grades. It's still made from the strong upper layer of the hide, but its surface is corrected for a cleaner look.

What is top-grain leather?
To make top-grain leather, the tanner takes the upper layer of the hide and lightly sands or buffs the surface to remove marks and scars. A finish, often pigmented, is then applied and sometimes embossed with an even grain pattern. The result is smoother and more uniform than full-grain.
One caution: the term isn't used consistently. Some sellers use "top-grain" for any leather from the upper layer, including full-grain. If the difference matters, ask whether the surface has been sanded or corrected.
How top-grain leather looks and feels
It looks clean and consistent, with few visible marks. It's usually a little thinner and more pliable than full-grain, and the finish makes it resistant to stains.
